POSITION SUMMARY
Under the direct supervision of the MAT MOUD Coordinator, the MAT MOUD Nurse provides skilled nursing services to incarcerated individuals with opioid use disorders. The MAT MOUD Nurse performs ongoing risk assessments, triage, teaching, inductions, stabilizations, medication administration and monitoring of patients in the MAT MOUD treatment program. Assists in the primary care of patients within
the MAT MOUD treatment program through the nursing process of assessment, planning, implementation and evaluation. Also directs and guides patient teaching activities and ancillary personnel while maintaining standards of professional nursing. Utilizes electronic medical record system as applicable throughout the scope of duties and responsibilities.
DUTIES/RESPONSIBILITIES
1. Provides MAT MOUD medication administration as well as observed dosing, providing injections and pill formulations.
2. Provides sound clinical judgment in decision making, understands patient needs, provides advocacy as needed and communicates nursing care to others on team.
3. Acts as liaison between patients and providers.
4. Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment.
5. Provides ongoing medication management for individuals in the MAT MOUD program.
6. Provides informed consent form medication administration or observed dosing.
7. Provides support to patients in all phases of treatment including nursing sick-call visits scheduled, chronic care clinics, and random urine toxicology screens, routine labs and as needed, medication teaching, monitoring, and pill counts.
8. Exhibits non-judgmental, empathetic, and supportive approach when communicating with patients and
staff.
9. Follows state and federal guidelines in providing care to opioid dependent patients in collaboration with licensed prescribing providers and pharmacy.
10. Consistently uses clear, concise, and effective written and oral communication; complies with departmental policies when sharing/documenting relevant patient care data.
11. Works with the pharmacies and the MAT MOUD team in obtaining medication orders, prescriptions, refills, prior authorizations, and communication as needed.
12. Determines, based on patient need, if patient needs to be seen by a provider and how quickly.
13. Promotes continuity of care within limits of staffing availability. Schedules urgent appointments appropriately.
14. Responds to patient sick-calls within a timely manner.
15. Documents all sick calls/visits; maintains accurate notes and attaches progress notes to patient's chart in a manner that is complete and easily understood by anyone who may need to access the record.
16. Meets with multi-disciplinary team on a regular basis and works jointly to establish expectations for how team issues will be managed and overcome barriers to treatment.
17. Promotes strict patient confidentiality and right to privacy.
18. Assesses physical, psychological and psychosocial dimensions of patients within the MAT MOUD program.
19. Applies knowledge of illnesses, injuries and diseases and use available resources during the assessment process to obtain health history.
20. Accepts staffing assignments and perform other related duties as required or assigned by the Director of Nursing/Health Services Administrator.
21. Develops and maintains an awareness of needed improvements in patient care, procedures, equipment and supplies and make recommendations to supervisors
22. As requested, assists in the presentation of in-service education.
23. Promotes quality assurance standards by participating in the quality of chart audits.
24. Maintains the confidentiality of inmate records.
25. Makes suggestions for improving work conditions to the Director of Nursing and Health Services
Administrator.
JOB REQUIREMENTS
The following requirements list the minimum to qualify. An equivalent combination of education and experience may be accepted.
LICENSURE: Current, unrestricted Louisiana Registered Nurse license required
CERTIFICATION: Current CPR Certification
EDUCATION: Graduate from an accredited school of nursing. Associate's Degree required
PREFERRED EXPERIENCE: Minimum of three (3) years of clinical experience preferred; previous experience working with individuals with opioid use disorders; correctional experience preferred
