**Position Summary:**We are seeking a compassionate and highly organized Care Manager to join our health care team. The Care Manager will be responsible for coordinating and managing care plans for patients, ensuring they receive comprehensive and individualized services. This role involves collaborating with healthcare providers, patients, and their families to facilitate optimal health outcomes while promoting patient independence and quality of life.
**Key Responsibilities:**
- Assess patients' needs, preferences, and health status to develop personalized care plans.- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and therapists, to coordinate patient care effectively.- Monitor patient progress and adjust care plans as necessary to address changing needs or goals.- Conduct regular follow-ups with patients and their families to ensure compliance with care plans and to provide education and support.- Serve as the primary point of contact for patients and families, offering guidance and answering questions regarding care options and resources.- Maintain accurate and detailed documentation of all care management activities and patient interactions in accordance with policies and regulations.- Advocate for patients to access necessary medical services, community resources, and support services.- Stay informed about best practices in care management and participate in continuing education opportunities.- Participate in case reviews and contribute to quality improvement initiatives to enhance the care management process.
**Qualifications:**
- Bachelor's degree in Nursing, Social Work, Health Care Administration, or a related field (Master's degree preferred).- Minimum of [2 years] of experience in a clinical or healthcare management role.- Strong understanding of patient care processes, social determinants of health, and healthcare resources.- Excellent communication, interpersonal, and problem-solving skills.- Ability to work independently and as part of a team, managing multiple priorities in a fast-paced environment.- Proficient in electronic health records (EHR) and other care management software.- Empathetic and patient-centered approach with a commitment to respecting the dignity and individuality of each patient.
