The Vermont Pathways to Partnership (VT P2P) is seeking dedicated individuals to support students with disabilities in becoming self-advocates and primary decision-makers in their futures regarding secondary education and employment. You'll collaborate with VT P2P team leaders, local schools, and partner organizations in the disability community to implement impactful regional programs and facilitate placement efforts for youth with disabilities.


Key Responsibilities:

  • Coordinate Services: Work with students with disabilities and their families to coordinate necessary services.
  • Data Collection & Presentation: Collect and present data to inform program development and outcomes.
  • Technical Assistance: Provide guidance and support to local education agencies.
  • Organizational Management: Ensure budgets and task details are meticulously organized.
  • Relationship Maintenance: Cultivate and maintain relationships with contacts at regional schools.

  • Excellent organizational and relationship-building skills.
  • Self-motivated with a passion for helping others succeed.

 

Location:
This position will be based in the Springfield area regional office, with significant fieldwork required across the region and statewide. A reliable means of private transportation is necessary.

Who May Apply

This position, VT Pathways to Partnership Transition Coordinator - Springfield - Limited Service (Job Requisition #50563) is open to all State employees and external applicants.  It is a Limited Service position, which is non-tenured and authorized for a specific period of time. Limited Service positions are established for specially funded projects or programs.

Environmental Factors

Duties are performed in both field and standard office settings. Considerable travel throughout the assigned district is required, for which private means of transportation must be available. Stressful situations should be anticipated, and incumbents must be able to persevere in difficult or problematic circumstances. Some work outside of normal office hours may be required by client needs.

Minimum Qualifications

Two (2) or more years of experience working in a human services field.
 

Preferred Qualifications

Associates degree or higher in human services, psychology, social work or special education.

Experience working with disabled youth.

Special Requirements

Candidates must pass any level of background investigation applicable to the position. AHS Policy 4.02, Hiring Standards, pursuant to 20 V.S.A. 2056c or other applicable statutory authority, requires criminal record checks for all AHS positions, including motor vehicle driving record checks and national record checks where appropriate.
