RVCC is looking to hire a Medical Case Manager! RVCC has a spectrum of services, including a robust HIV/AIDS Project. This program provides comprehensive, bilingual and bi-cultural case management, mental health counseling, peer support and other services for HIV/AIDS positive people and their families.# River Valley Counseling Center, Inc#s Mission is to improve the health of all people in our community. We do that through expert and compassionate care, education, community partnerships, fostering innovation, and by inspiring hope in all we touch.# We do that by being good stewards of our resources and providing efficient and cost-effective care for all. # Overview of Job Responsibilities: To obtain required documentation from clients.# Complete initial Acuity Assessment with timely Acuity Re-assessments. To competently provide information and referral services for consumer services based on assessed client needs. To oversee the development, implementation, and monitoring of accuracy and cognitively appropriate.# Individual service plan developed in agreement with program participant(s), as required by funders. To provide individuals and their families in completing applications or various paperwork related to life management; i.e; medical coverage, housing, disability, etc.; when necessary and in a timely manner. To provide or arrange for the provision of appropriate support services to assist program participants in maintaining independent living, such as budgeting, accessing parenting skills training, or other skills guidance, etc.; as appropriate. To provide outreach services to assist program participants in using and accessing needed community and medical services, including arranging transportation. To competently provide education to program participants and families/significant others concerning AIDS/HIV transmission and risk reduction in accordance with appropriate related guidelines, to refer for PeEP and partner services as needed. To advocate for and provide support to individuals and families Job Requirements: Education Level -#CHW Certification, Associates or Bachelor#s Degree with a concentration in human services preferred.
