Dialysis Travel Social Worker
• Responsible for driving company culture through values and customer service.
• Assessing patient understanding of kidney disease and identifying any barriers affecting adherence to treatment.
• Assessing patients psycho-social status, strengths and areas of need.
• Providing supportive and goal directed counseling to patients who are seeking transplants.
• Participation in collaborative care planning with the patient and healthcare team to assist patient in meeting treatment goals while improving quality of life.
• Assessing patient awareness of advance directives and DNR status.
• Providing ongoing support to patient and family regarding psycho-social issues related to end stage renal disease.
• Developing and training staff as needed regarding psycho-social topics related to dialysis patient care.
Job Requirements
• 1+ year of recent experience as a dialysis social worker
• Master's Degree in Social Work from a school of Social Work accredited by the Council on Social Work Education (CSWE)
• Current state social worker license required
• Experience with renal patients preferred
• BLS certification required
