We Are Hiring: Social Worker - Inpatient Behavioral Health - Waukesha Memorial Hospital - Full Time
ProHealth Care is seeking a dedicated Social Worker to join our Inpatient Behavioral Health team. In this role, you will assist patients and their families in navigating the inpatient psychiatric hospitalization process. Collaborating with a multidisciplinary clinical care team, your primary responsibility will be to connect patients and families with appropriate internal and external resources, including community services and support groups. Additionally, you will provide one-on-one psychosocial support to patients and their significant others who require assistance beyond the clinical visit.
Schedule Details: Schedule: Business hours, 8 AM - 4:30 PM
What You Will Do:
- Receive referrals for voluntary and involuntary clients with various mental health diagnoses.
- Participate in interdisciplinary team meetings to develop patient treatment plans.
- Facilitate safe and, at times, complex discharge plans.
- Coordinate care with community providers.
- Assess social determinants of health (SDOH) and provide relevant resources.
- Ensure documentation meets organizational core measures.
- Assist with FMLA and short-term disability forms.
- Handle insurance-related tasks.
- Collaborate with Waukesha County Corporation Counsel for involuntary patients (e.g., chapter holds, 3-party petitions, commitment order revocations).
- Exhibit excellent verbal and written communication skills.
- Work collaboratively with the multidisciplinary team.
- Identify and recommend appropriate resources to meet patients' needs, adhering to the ProHealth Care Experience.
- Maintain awareness of community resources and services, continually exploring partnerships and support groups.
- Complete all documentation accurately and efficiently in a timely manner.
Qualifications:
- Credentialed provider with an Accountable Care Organization (ACO).
- Master's degree in Social Work or related field.
- Licensed Clinical Social Worker (LCSW) preferred; consideration given to Advanced Practice Social Workers (APSW) seeking supervision for clinical licensure.
