What You Will Do:
ProHealth Care is looking for an Advanced Practice Provider (NP or PA) who takes an active role in identifying and creating opportunities to educate, motivate and collaborate with staff and students in their professional development. PHC is looking for someone who renders advanced clinical practice services to all ages under a collaborative agreement with a designated PHMG physician. Responsibilities include: assessment, diagnoses, treatment and when necessary referral to others for further care.
Responsibilities
- Assessment, diagnosis and treatment of patients under their care. Conditions managed include both acute and chronic illnesses with management of co morbidities in the ambulatory care setting.
- Obtains medical histories and ensures relevant documentation in accordance with PHMG standards, the state of Wisconsin, PHMG Scope of Practice and collaborative agreement with PHMG physician.
- Interprets and evaluates patient care data in order to determine management and treatment of diagnoses in accordance with evidence based practice, current clinical guidelines and medical processes. Orders and interprets diagnostic studies in the field of trained clinical specialty. Performs minor surgeries and routine procedures.
- Encourages patient involvement in care through counseling, instructing and educating. Promotes active discussion with patient regarding plan of care, treatment options, care outcomes, preventive health measures and self-care skills.
- Provides coordination of referral services including managing a variety of ancillary services, community resources and preoperative plans.
- Prescribes, recommends and regulates both pharmacologic and non-pharmacologic therapies including controlled substances, as warranted for patient specific conditions.
- Adjusts patient plan of care by monitoring treatment plans and patient outcomes.
Requirements
- Minimum Education Level - Master's degree in Nursing
- Licensed Nurse Practitioner Required
- Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) in Family Practice or Adult Primary Care
- Certifications: NCCPA, DEA, CPR (BLS and/or ACLS)
