At NorthBay Health, the Population Health LVN works closely with the Population Health RN Case Managers or Population Health RN Inpatient Case Managers to provide care in accordance with best practice guidelines. This individual will play a key role in optimizing patient health and wellness, reducing inpatient admissions, minimizing re-admissions, and improving medical stability within the patient population. This position works closely with Case Managers and other care team members to manage complex patient conditions and high-risk disease management.

Qualifications
Education: Completion of accredited LVN Program.
Licensure/Certification: Current California LVN license. Current AHA or equivalent certification in CPR. Current Driver's License in the state of California.
Experience: Minimum of two years of experience in Case Management and/or Population Health.
Skills: Demonstrated effective time management skills. Must demonstrate and maintain current knowledge and skill in providing ambulatory nursing care as appropriate for patients in the following age groups: Geriatric, Adult, Young Adult, Pediatric and Neonatal. Excellent verbal and written communication skills. Strong organizational skills with the ability to manage multiple tasks and prioritize effectively. Proficiency with Electronic Health Records (EHR) and basic computer skills.

Hours of Work: Based on business need but mostly works Monday through Friday between the hours of 8:30 and 5:30
Other: Bilingual preferred. May require occasional travel to different locations.
Compensation: $37.51 to $43.45 based on years of experience doing the duties of the job.
