Overview:
To assist clinic patients to access services and referrals as needed. To follow clinic policies and procedures for all financial and adherence requirements.
Qualifications:
- Education: Bachelors degree in Social Work required.
- Experience: Minimum of one years experience in a health care setting with Hospice experience preferred. Experience with issues related to bereavement and the terminally ill patient preferred.
- Other: Good oral and written communication skills; Good organizational skills. Must be a licensed driver. Ability to work as a member of an interdisciplinary team.
- Preferred Education: Masters degree in Social Work.
- Preferred Licensure: LMSW or LCSW in the state of employment.
