Job Summary: 

MedStar Health is looking for a Social Work Team Lead Inpatient Case Manager to join our team at MedStar Franklin Square Hospital.  

As a Social Work Team Lead Inpatient Case Manager, you will work with the manager to help the Case Management Department's implementation, integration, and operational direction. Facilitates the continuum of care, works collaboratively with the interdisciplinary staff, both internal and external to the organization. Assists in ensuring that specific disciplines are represented in the department's decision-making process. Acts as a resource and mentor.

Primary Duties: 

  • Provides professional expertise as a vital member of the interdisciplinary team to ensure that patients receive the most appropriate and cost-effective care. Collaborates with interdisciplinary team in Identifying barriers that may result in delays in discharge and develops strategies to minimize; communicates successful strategies with the team. Provides leadership, instruction, supervision, and direction to associates in the Case Management Department.  

  • Supervises, monitors, and documents professional progress of social work staff as required by state licensing statuses. Participates in ongoing training and professional development opportunities to stay current on best practices and new developments in utilization management. Assists with selection, training, orientation, and assignments of the department staff. Assists with development of performance standards, and evaluation of associate performance. Initiates/provides recommendations for personnel actions. Maintains ongoing communication with associates to review progress, provide feedback, discuss new developments, and exchange information.  

  • Assesses staff professional development and training needs. Participates in staff meetings, Care Management meetings, work groups, and other in-services to enhance professional knowledge and competency in management of patients. Collaborates with Primary Care Physicians, Medical Specialists, Home Health, and other ancillary providers to coordinate patient care. Provides professional expertise as a vital member of the interdisciplinary team to coordinate transition management for patients and arrange for appropriate care to ensure safe and timely discharge. Identifies barriers that result in delays in transition and develops strategies to minimize; communicates successful strategies with team. Effectively balances patient safety and discharge needs with hospital length of stay goals.  

  • Demonstrates knowledge of resources available in the system and community and utilizes them effectively in supporting the staff and patients during care. Advises providers and staff on availability of appropriate services for patients who require post hospital care. Identifies training needs related to the clinical software system (data entry/documentation) and programs or processes within the Case Management Department. Implements a plan for training based on identified needs. Provides support to manager's responsibilities and deliverables, including the Case Management Department Plan and Appraisal of LOS, IMM and MOON delivery. 

  • Successfully completes 2-3 preceptor and/or staff education assignments per year, following established education plans and guidelines. Provides insight and suggestions for improvement. Regularly reports progress of new associates to their manager and actively participates in planning and follow-up meetings regarding associate progress. Assists with departmental projects and other functions such as support department operations and/or assist with patient specific issues that may arise. 

 

Qualifications:  

  • Master's degree in social work (LMSW) or master's degree in LGSW (DC hospitals only). 

  • 2 to 3 years Acute Hospital clinical experience. 

  • Valid LMSW/LGSW license in state/district of employment, or Licensed Certified Social Worker (Clinical LCSW-C/LICSW) if supervising. 

  • 1-2 years' Social Work experience preferred.  

  • 1-2 years' experience in case management preferred.  

  • 1-2 years' progressive leadership experience preferred. 

  • LCSW-C/LICSW if not supervising preferred. 

  • Certified Case Manager preferred. 

 

This position has a hiring range of $72,758 - $123,260
