Job Summary:
Medstar Health is looking for a Patient Navigator/ Community Health Educator to join our team at Fort Lincoln Family Medical Center. The ideal candidate will possess experience in a health care environment and strong verbal/ written communication skills.
The Patient Navigator/ Community Health Educator responsible for coordination and facilitating access to health-related services for clients by utilizing available resources.

Primary Duties:
Assists staff in promoting health education and quality improvement projects. Participates in orientation, training, and education programs for students, residents, physicians, nurses, and other health care professionals as required. Works with program team to ensure grant activities are on track, managed, and completed. Attends project meetings related to grant activities. Participates in multidisciplinary quality and service improvement teams as appropriate.
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Ensures compliance with hospital/facility policies and procedures and governmental/accreditation regulations.
Develops and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (healthcare members). Identifies potential clients, completes initial needs assessment and maintains follow-up to ensure appropriate guidance to health services.
Screens target populations for eligibility. Reconciles care gaps for target populations; communicates with external stakeholders (e.g., health plans) to correct discrepancies. Maintains accurate client records by documenting all contacts, services provided, and outcomes to track all clients and services for future reference in Electronic Medical Record (EMR) and database registry of target populations.
Supports client self-management goals by listening to client/family concerns, assisting with problem-solving, assessing how well the self-management plan is helping the client reach their goals and encouraging individualized goal setting and revision. Assists clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, assisting with the completion of medical program applications, and managing complex health referrals.
Qualifications:
High School Diploma/ GED with 2 years of experience in a healthcare environment; or bachelor's degree.
Strong understanding of cultural competency with target population.
Master's degree in public health or related field preferred.
