Description

The licensed clinician will serve as a resource to the nursing associates, physicians and patients regarding glycemic and diabetes education for inpatients; acts as the coordinator of the Glycemic Control Program and provides education, psychological and emotional support to patients and families. The ages of the patients served are infancy through adulthood. In addition, the clinician will support the Bariatric Program by accurately abstracting and entering data into the MBSAQIP data registry in a timely manner. The clinician will collaborate with the bariatric team to help identify opportunities for clinical improvement.
Education

  • Bachelor's degree BSN required and
  • Maryland license if applicable. required
Experience
  • 5 years of experience in a clinical setting required
Licenses and Certifications
  • CDE - Certified Diabetes Educator Certification Certified Diabetes Educator preferred or has qualifications to take the national exam within two years of hire and obtain credentials. required and
  • Successful completion of the MBSCR Online Training Program within 1 year of hire. within 1 Year required

  • Evaluates and treats diabetic patients using appropriate assessment tools and communicates findings via written and oral report. Assess and evaluate patients admitted with abnormal glucose values. Reviews H&P, med rec, blood pressure, weight, lab values, tobacco and substance use, overall health status and mental health status per program protocols.
  • Is proactive in seeking the latest information about practice and theory in the area of glycemic care and diabetes and its complications and shares information with the multidisciplinary team. Consults with physicians and the multidisciplinary team regarding orders, diagnostic tests, medication management, dosing and adjustments as needed for appropriate treatment based on individual patient care needs.
  • Coordinate care with dietitians to meet nutritional support and hydration. Ensures documentation entries are completed in chart; including process implemented, plan of care and patient response to interventions. Ensures continuity of care through participation in inpatient discharge planning and follow-up as necessary to ensure successful transition to home or other placement.
  • Serves as coordinator for the Glycemic Control Committee providing leadership and encouraging multidisciplinary collaboration. Collaborates and participates in the MedStar Diabetes Institute serving as representative for MMMC. Reviews and reports glycemic tracking and quality improvement outcome data.
  • Delivers program education to multidisciplinary team. Identifies learning needs of multidisciplinary team members and provides coaching/ mentoring as necessary. Develops educational plans, coordinates appropriate resources, evaluates response to teaching and follows up with associates to evaluate outcomes. Implements educational programs in a knowledgeable, clear, and understandable fashion, utilizing adult teaching strategies.
  • Delivers program education to patients. Assists with the assessment of the patient/family educational, emotional, and psychological needs and ensures that these needs are met through individual educational sessions.
  • Counsels patient/family on the aspects of glycemic and diabetes care regarding nutrition, psychosocial needs and physical activity. Makes recommendations for continual care such as diabetic education, home health or counseling.
  • Counsels individual patients regarding exam findings, abnormal lab results, appropriate follow-up, any implications for personal health and the impact on job performance and/ or daily activities.
  • Works with patients to optimize understanding of medications and to develop strategies to promote medication adherence. Recommends prevention and wellness education programs for patients/ caregivers and / or family members.
  • Metabolic and Bariatric Surgery Data Abstraction. Accurate and timely abstraction and submission of bariatric cases into the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) data registry platform. Collects preoperative, intraoperative, postoperative, and long-term follow-up data components through effective utilization of hospital medical record system.
  • Retrieval and entry of long-term follow up data over time. Collaboration with the Bariatric Program Coordinator, medical staff, surgeon's office, and clinical departments to identify quality improvement opportunities. Participation in accreditation site visits. Active participation in Bariatric Program committees and initiatives.
  • Maintains current knowledge, certification, and licensure. Identifies learning needs in self and initiates actions to address these education needs. Attends continuing education programs in order to maintain licensure and certification.
  • Attends MedStar Health and MedStar Montgomery Medical Center sponsored training programs as required by the department. Completes all mandatory requirements, certifications, and licensure within allotted timeframe for completion (License, PPD, Competencies, Age Specifics, etc.)
  • Works effectively within and between departments. Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate
  • Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees. Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit.
  • Department/Unit Specific Functions and Duties Attends continuing education programs to keep current with diabetes education practices: CDE- 75 hours of approved continuing professional education during the five-year recertification cycle b. Performs other duties as assigned.
