Job Summary:

MedStar Health is looking for a Diabetes Care & Education Specialist to join our team at MedStar Good Samaritan Hospital's Good Health Center/Collaborative Care Program.

As a Diabetes Care & Education Specialist, you will deliver comprehensive and seamless services that bridge the gap between the clinical and self-management aspects of diabetes care for our patients. By providing nutritional counseling and insulin management services in conjunction with the Collaborative Care Program team.

Certified Diabetes Care and Education Specialist - CDCES Certified Diabetes Care and Education Specialist highly preferred.

Primary Duties:

·         Provides diabetes education and management services to patients, including initial patient assessment and ongoing care planning to optimize patient success.

·         Provide insulin pump and continuous glucose monitoring device education and assistance to patients utilizing the devices.

·         Provides quality diabetes self-management education and medical nutrition therapy based on assessed needs in individual and group classes which are in-person or via telehealth.

·         Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient' assessed needs and goals and promotes successful self-management.

·         Collaborates, advocates and confers with nurse practitioners, primary care providers, endocrinologists and other diabetes stakeholders in developing patient centered diabetes plan.

·         Provides non-diabetes medical nutrition therapy to referred patients if there if schedule allows.

Qualifications:

·         Bachelor's degree in Nutrition/Dieteticsrequired.

·         Registered Dietitian license – Maryland required.

·         1-2 years of related experience as a registered dietitian required.

·         Diabetes self management education and support experience preferred.

·         CDCES - Certified Diabetes Care and Education Specialist Certification; if not already CDCES will sit for the certification examination within 12 months of hire date.


This position has a hiring range of $72,758 - $130,041
