General Summary of Position

MedStar Health is looking for a Community Health Advocate to join our team at MedStar Georgetown University Hospital! The ideal candidate will possess knowledge of local social service resources, the ability to make connections, promote healthy behavior change, facilitate learning opportunities, and effectively build partnerships with community organizations and key leaders. 

Under the direction of the Manager of Community Health Outreach, the Community Health Advocate (CHA) works closely with case management, community, and social service agencies to provide connection to resources and programs for community members to improve their health and general well-being. Primarily working in a community-based setting, the CHA serves as a link between health/social services and the community to facilitate access and improve the quality and cultural competences of service delivery. In addition, the CHA builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities including outreach, community education, informal counseling, social and navigation support, health screenings, assessments, and advocacy.

Primary Duties: 

  • Conducts and supports execution of Community Health Needs Assessment (CHNA) through coordinating community health programming. Collects, tracks and reports data on community health programs and community benefit activities for the hospital  

  • Works to build partnerships with key community organizations, leaders, and hospital departments. Participates in community meetings, serves on internal committees, and represents the department and hospital in community outreach efforts as appropriate. 

  • Communicates to community members the purposes and potential impact of the community health programs on their well-being. Assists community members in identifying socio-economic issues that affect their overall health and develop health/social management plans and goals. 

  • Connects community members with health-related services and other community resources to address the social determinants of health. Motivates community members to be active and engaged participants in their health and overall well-being. 

  • Enhances community members' ability to effectively community with health providers. Advocates for health equity for the communities served by the hospital. 

 

Qualifications:  

  • High School Diploma or GED. 

  • 1-2 years of experience working with individuals/families and/or communities in outreach programs. 

  • Valid State Driver's License in good standing; Reliable, dependable, and insured transportation. 

  • Associate or bachelor's degree preferred. 



This position has a hiring range of $48,235 - $80,059
