Position Summary Assesses, plans, coordinates, and evaluates options and services to meet an individual and family#s comprehensive health needs through communication and available resources to promote quality cost-effective outcomes. Including, the implementation of individualized services to meet the needs of patients and their families. Ensures all actions taken in carrying out these responsibilities in support of patient centered care. Position Responsibilities Unit Specific Position Responsibilities Practices ethically. Communicates effectively in all areas of practice. Facilitates collaboration between patient, family, and the interdisciplinary team in the provision of therapeutic practice. Remains knowledgeable in and serves as a resource person for the various current healthcare/community resources available, insurance requirements, and funding sources. Maintains professional competence. Continually reassesses competencies and identifies needs for additional knowledge, skills, personal growth, and integrative learning experiences Utilizes appropriate resources to plan and provide services that are safe, effective, and financially responsible. Respects diversity and individualizes care to meet the unique needs of the patient. Establishes partnerships with patients, families, support systems, and other providers, utilizing verbal and electronic communications to deliver health care services. Assesses and identifies current, potential, and ongoing psychosocial and transition care needs of patients and families. Analyzes the assessment data to develop the patient#s problems list. Identifies individualized goals/outcomes and coordinates services to meet these goals/outcomes. Develops, implements, evaluates and revises the individualized plan of care as needed with the assistance of the interdisciplinary health care team. Coordinates activities and documentation related to Court Committals. Provides education and supportive interventions in a way that builds a relationship between social worker and patient. Coordinates the discharge planning process. Assists patient/family in determining/acquiring financial resources for payment of clinical services. Attends and participates in the multidisciplinary discharge planning meetings. Maintains timely and appropriate documentation of assessment, individualized plans of care, discharge plan, coordination of activities and progress within the plan. Maintains and improves the healthcare practice environment conducive to the provision of quality health care. Integrates evidence and research findings into practice. Practices in an environmentally safe and healthy manner. Collaborates with various external health care members in coordinating various services to meet the patients# needs. Performs related work as required. Provides a comprehensive behavioral health screening of patients presenting in the Emergency Department or referred by external referral sources. Uses the screening data and information to report findings to the Emergency Department physician or psychiatrist/designee. Performs de-escalation and violence management interventions to manage physical/verbal aggression by patients. Performs de-escalation and violence management interventions to manage physical/verbal aggression by patients. Qualifications, Knowledge # Experience Required Qualifications (Including any licensure, certification, education): Bachelors Degree in Social Work or related field. Mandatory Reporter training within 90 days of hire and must maintain throughout employment. Combative Patient Response Team training within 90 days of hire and must maintain throughout employment. De-escalation training prior to performing patient care or within 90 days of hire. Organizational Requirements: Maintain stroke education per regulatory requirements. # Preferred Qualifications: Two years of social work/case management experience in a health care setting. # Required Knowledge, Skills # Experience: Skill in effective oral and written communication in English. Knowledge of basic arithmetic and its applications. Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. Active Listening skills, giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. Ability to manage one#s time effectively. Ability to pursue and foster a sense of team and partnership across all disciplines.
