Job Summary:
Provide integrated and coordinated case management services for the complex, high risk and high utilizer populations in variable psychiatry, mental health, and addiction medicine settings. Responsible for managing facets of care and utilization for identified complex psychiatric, mental health, and addiction medicine patients.

Essential Responsibilities:

  • Engages with high risk, high utilizing behavioral health patient populations

  • Initial intake and intervention with patient following referral

  • Evaluate the goals and needs of patients from targeted treatment plans

  • Execute on tasks identified in the individualized clinical care plan

  • Routinely assesses for psychosocial gaps

  • Navigates and advocates for patient to clinically appropriate and medically necessary levels of care

  • Coordinates, provides and/or facilitates informational classes, education and/or training in areas such as independent living skills, psychosocial education

  • Assists with member activities of daily living as it applies to the individual care plan

  • Works and communicates with multi-disciplinary team to manage care

  • Forms completion and management

  • Utilization of clinical and electronic tools within the EMR

  • Provides assessment and evaluation of clinical status, environmental resources, mental and intellectual capabilities and networking skills during the intervention period

  • Planning of services based on assessment of both short and long-term needs

  • Tracks referrals and outcomes for ongoing quality management

  • Facilitates collaboration between Primary Care, Behavioral Health Care Services and Health Care Providers

  • Provides referrals to other departments; facilitates this process and may provide follow-up services as appropriate

  • Consults with families and other care givers

  • Provides service information, educational materials and resource information

  • Facilitates scheduling of multi-disciplinary assessment team and may participate in assessment

  • Linkage of member with external and other community-based resources

  • Establishes linkages and maintains working relationships with outside resources

  • Provides crisis intervention as needed

  • Consistently supports compliance and the Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures.

  • In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives.

  • Also accountable for consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors.

Basic Qualifications: Experience
  • Relevant experience from one of the following areas shown below:
    • Behavioral Health related training or clinical rotation in certified educational program
    • Recent experience in case management and working with community resources
Education
  • Masters Degree in applicable Mental Health field.
License, Certification, Registration
  • N/A
Additional Requirements:
  • National Provider Identifier (NPI) and Taxonomy Code Required at hire.
  • Knowledge of Psychiatry and Addiction Medicine patient populations in inpatient and outpatient settings
  • Knowledge of the complex systems of psychiatric care and substance abuse treatment at the community and county levels
  • Outstanding communication and organizational skills.
  • Consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to colleagues, members, customers, contracted providers, and vendors.
  • Working knowledge collaborating with team members and family members, accessing internal and external resources.
  • Working knowledge of data collection tools, databases and word processing programs.
  • Demonstrated customer service skills, customer focus abilities and the ability to understand Kaiser Permanente customer needs.
Preferred Qualifications:
  • Bi or Multi lingual.
