- CA state healthcare exclusion list. Medi-Cal Program eligibility completed within 30 days prior to first assignment and monthly thereafter.

- HLC Testing to be completed by Friday before Start. Candidates Username and Password will be provided by CMC under the candidates comments. If completed and confirmation provided Friday prior to start, a $300 stipend will be paid.

- Primary Source Verification of professional license, Nursys, and/or certification, as required for the position ran within 30 days of start updated upon expiration.
- Additional Details
- Proof of vaccine and 1 booster or exemptions must be submitted for approval.
- Proof of 2 immunizations, positive IGG titer, if negative titer, must have booster or Declination accepted.
- Will be completed onsite with CHS on start date
- Proof of 3 immunizations and positive antibody titer, if negative titer, booster or Declination accepted.
- Proof of vaccine from October - March (unless otherwise indicated by CDPH) or declination.
- Annual negative TB skin test or QuantiFERON Gold or T-Spot. Must state negative results. If positive, must show proof of positive history, initial Chest x-ray and annual TB Questionnaire thereafter.
- Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
