CHI Health would love to add a Virtual Clinic RN to our team! This position is for hospital discharge calls for approximately 6 primary care clinics across NE!
****You MUST live within 50 mile radius of Omaha, NE! Will NOT accept out of state applications. There will be a requirement of in-person training AND in-person meetings.**
Hours: Monday-Friday between 8a-5p, NO weekends, NO holidays & NO call!

**Responsibilities**
**Essential Key Job Responsibilities**
Provides virtual patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
Accepts inbound communication such as calls from patients, pharmacies and other healthcare entities.
Performs outbound calls to notify patients of reviewed lab and imaging results according to protocols that require review of the patient chart to answer questions.
Performs telephone triage utilizing evidence based triage protocols.
Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
Follows medication refill protocols and acts accordingly to those protocols.
Ensures accurate patient documentation.
Performs other duties within scope of role as assigned.
**Qualifications**
**Minimum Qualifications:**
Current Registered Nurse (RN) license that allows you to practice in our state
Graduate of an accredited nursing program
Basic Life Support (BLS) from the American Heart Association required
**Pay Range**
$26.38 - $38.26 /hour
