**JOB DESCRIPTION**

The Transplant Coordinator is a Registered Professional Nurse with specific knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations within the Transplantation Institute.

**RESPONSIBILITIES**

**Responsible For**

Development, implementation, coordination and evaluation of clinical programs and services for designated patient populations.

**Principal Duties and Responsibilities**

**Clinical Practice**

1. Coordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites. The multidisciplinary team includes the transplant surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy and finance. Coordinators work with their assistants to facilitate other medical specialty visits as needed.

2. Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required (including bloodwork sent to Rogosin for tissue typing).

3. Participates in making recommendations regarding placing as patient on the transplant waiting list. Educates patients on the benefits of living donation (when applicable).

4. Communicates with all active transplant candidates regarding waiting list status on an ongoing basis. Ensures all medical clearances are updated regularly.

5. On call responsibilities include facilitating all organ offers from UNOScommunicating with surgeons, fellows and patients to ensure timeliness of all organ offers. Coordinators also answer patient calls after hours, on holidays and weekends.

6. Follows patients during their hospitalization and coordinates with transplant team to coordinate consulting services as needed. Coordinates outpatient follow up visits with the outpatient transplant team.

7. Educates patients and family regarding all transplant medications, lifestyle changes, as well as outpatient follow-up protocols.

8. Assesses continued recovery of patient through compliance with outpatient visits, scheduling of blood tests, regular review of labs, adjusts immunosuppressive medication as needed during regular lab review.

9. Considers ethical issues in professional nursing practice.

10. Treats co-workers, patients and families with dignity and respect.

11. Is open and responsive to the diverse backgrounds and experience of other people, and promotes and environment that is sensitive to cultural diversity.

12. Maintains attendance and punctuality record in accordance with departmental standards.

13. Adheres to dress code in accordance with departmental policy.

**Administrative Practice**

1. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.

2. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards.

3. Participates in the development and implementation of continuous quality improvement programs.

4. Participates in risk management activities including identifying and communicating issues of risk in a timely manner.

5. Participates in committee activities.

6. Is aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources.

7. Recognizes each individual's right to health care unrestricted by age, race, color, sex, age, national origin, disability, sexual orientation or source of payment.

8. Assists in the recruitment/retention, annual evaluations and disciplinary process for support staff of their designated organ program (i. e. transplant coordinator assistant and medical assistants).

**QUALIFICATIONS**

NYS RN with BSN and BCLS with at a least 2 years acute care nursing experience. Critical care experience preferred.
