**General Summary**
Responsible for leading an interdisciplinary clinical team providing patient care. Supervision of services provided to patients in their home atmosphere, choosing and developing clinical staff/self, able to take an active lead in management of emergencies, actively participates in quality improvement and safety. Also markets and carries out activities to promote customer relations and agency growth. Participates in Home Care community service or community benefit activities, actively participates in Home Care initiatives working with Director(s) to achieve these goals, responds to patients and families in a manner that reflects professionalism and that demonstrates our Mission and Values.
**Essential Job Functions**
-Communicates with clinical and office staff to ensure scheduling is cost effective, efficient and set to provide care leading to optional outcomes. Oversees staffing and visit assignments, coordinating patient care, coordinating referrals, and assuring the development, implementation and updates of the individualized plan of care. Works with clinical and office staff to provide ongoing field staff supervision, coaching and mentoring in the form of supervisory visits (annual, orientation). Acts as a resource to staff.
-Ensures adequate and proper staffing ratios to adjust clinical staff on daily basis to assure correlation of staffing with workloads and volumes. Collaborates with Director(s) to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of health personnel.
-Coordinates with referral sources, staff, and other appropriate personnel to ensure the admission and continuity of care for home care patients. Receives, or directs the receipt of, information from referral sources and physicians related to the admission, start of care, and continuation of patient care.
-Leads and evaluates ongoing care of patients through interdisciplinary/case conferences. Ensures the patients' plans of care are developed, implemented and evaluated. Actively pursuing proper utilization, discharge planning, progress toward goals, and problem solving. Investigates and reports any problem relating to patient care or conditions which may harm the patient and/or employee well being.
-Supports/assists the Quality department with reviewing of required documentation. Collaborates with Quality department to evaluate performance of clinician documentation. Works with clinician on documentation education and improvement if warranted. Monitors timely submission of documentation and ensures timely resolution of corrections to documentation.
-Provides on call coverage as scheduled addressing administrative and agency needs appropriately and independently.
-Responsible for recruiting, hiring, evaluating, engaging, retaining and terminating service personnel. Responsible for completion of accurate and constructive performance evaluations and supervisory visits in order to develop a high quality clinical team. Proactively monitors team and individual performance. Provides coaching, mentoring and action plans for team members. Promotes and monitors staff participation in appropriate educational programs. Expands knowledge base through self-development. Responsible for Human Resources functions and corrective action processes. Continually updates own knowledge and serves as information resources for staff.
-Serves on the QAPI Committee. Actively participates in quality improvement QAPI and safety. Conducts investigation of adverse events for OASIS outcomes (as delineated by CMS). Proactively monitors industry trends, agency deficits and suggests initiatives/changes in practice to support continuous QAPI. Give input and implement improvement activities. Initiates, investigates and assists with evaluation of patient/staff incidents; complaints. -Independently works to resolve complaints. Involves quality and risk administrative personnel for incidence of compliance, risk, liability. Presents and evaluates findings to Director/Administrator as appropriate. Initiates infection control reports. Conducts chart audits/reviews as a participant of QAPI program. Assumes QAPI projects, analyzes data, and completes them accurately and independently in a timely manner. Reports data and progress on QAPI projects regularly.
-Participates in planning for orientation of new employees and education of existing employees. Conducts selected orientation/education classes as needed. Collaborates with Quality and Education to develop education plans, competency evaluations, and additional educational needs for the agency.
-Demonstrates leadership and team building skills. Has the ability to engage and lead a team. Acts as a change agent for the organization and is responsible for implementation. Works with Director(s) in identifying budgetary requirements. Demonstrates knowledge and understanding of adhering to the operating budget, controlling and reducing cost when possible. Holds team accountable to productivity standards set forth by the agency. Participates in developing objectives for the agency. Displays a willingness to support the policies and procedures and follows appropriate channels for change. Leads regularly scheduled team meetings, in-services, case conferences and maintains documentation. Ensures compliance with federal, state, accrediting bodies, local, and agency polices/regulation.
**Required Minimum Education:**
2 Year/ Associates Degree

Registered Nurse licensed in Ohio, CPR, Current State driver's license
