Are you an experienced Oncology Registered Nurse looking to take the next step in your career? UPMC Altoona is seeking an RN Oncology Patient Navigator to support the Radiation Oncology department! This position will work daylight hours, Monday through Friday.

The Oncology Patient Navigator is responsible for providing assessments, counseling, communication facilitation, intervention and coordination of the identified oncology patients through the cancer diagnosis and treatment process.

**Responsibilities:**

Demonstrates a knowledge of reimbursement systems, the ability to educate patients and families regarding payer requirements/coverage for post-hospital services, and the ability to effectively advocate on behalf of the patient to obtain authorization.

Demonstrates appropriate professional practice, maintaining respect for confidentiality and freedom of choice

Risk Management:

Able to identify practices and/or events that jeopardize patient safety.

Report to hospital attorney, Risk Manager or Quality Assurance Director as needed.

Documentation and Quality Assurance:

Able to identify appropriate standards of care.

Conducts concurrent case reviews

Monitors for quality issues

Makes appropriate referrals to Physician Advisor or Risk Manager when standards of care not met.

Via telemedicine-provide coordination of services including scheduling, patient assessment and follow-up as deemed necessary.

Identify, assess, counsel, and effectively intervene with medically and psychosocially complex patients.

Educates and advocates for patients and families to enhance their ability to make informed decisions regarding medical care, discharge planning, advance directives, and end of life care.

Maintains extensive knowledge of federal, state, and local assistance programs and community resources that affect patient needs; demonstrates expertise in accessing social systems by providing resource-specific information to patients/families, initiating contact with resources and facilitating the patient's/family's ability to accept referrals.

Risk Management:

Able to identify practices and/or events that jeopardize patient safety.

Report to hospital attorney, Risk Manager or Quality Assurance Director as needed.

Completes a psychosocial assessment that includes social, economic, cultural, age-related and behavioral factors, history of child, elder, or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems, and all other available resources.

Provides accurate, timely and appropriate documentation of interventions and outcomes in the appropriate record keeping system.

Collaborates with the healthcare team, patients and families, and relevant community resources to ensure the oncology patient understands and actively participates in their plan of care. Monitors and evaluates plan, options, and available services to effectively meet the patients' needs.

Bachelor's degree in Nursing with minimum of seven to ten (7-10) years' clinical experience.

Oncology experience required.

Case Management experience preferred in addition.

OCN certification preferred.

**Licensure, Certifications, and Clearances:**

Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
