**Day in the life of a Hospice Case Manager:**

Care Coordination: Coordinate and oversee the holistic care plan for hospice patients, ensuring it meets their physical, emotional, and spiritual needs.

Patient Advocacy: Serve as the primary point of contact for patients and families, advocating for their preferences and ensuring their wishes are respected throughout their journey.

Team Collaboration: Collaborate with interdisciplinary teams including physicians, nurses, social workers, and spiritual counselors to provide comprehensive care.

Documentation and Compliance: Maintain accurate and up-to-date documentation of patient care plans and interactions, ensuring compliance with regulatory standards and hospice policies

**Current Openings:** Full-Time Monday-Friday, Back-up on-call Weekday and weekends

**Compensation:**

The pay range for this position is $72,483 - $90,603 / year. The starting pay rate will be based on years of experience.

Mileage Reimbursement at 58.5 cents per mile

Active RN license in state of Iowa
