Summary The Transfer Coordinator RN is a Registered Nurse, qualified by experience and education to provide direct staff support in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and/ or services to improve the quality of patient care and the appropriateness and timeliness of services provided. Responsibilities The Transfer Coordinator is aligned with the department of Nursing and collaborates with all services to ensure safe transfer outcomes. The Transfer Coordinator is responsible for providing the clinical leadership and expertise for the coordination of the medical center's Transfer Center Operations. He/she possesses intimate knowledge of federal regulations, community regulations, EMTALA, VHA guidelines, medical review criteria, utilization management processes, clinical documentation requirements, current standards of care, direct patient care, to include incoming/outgoing consult completion, initiates and tracks emergency care notifications, follows emergency medical transport procedures, and compliance guidelines. Clinical oversight of the non-VA care approval process is provided by the medical center Chief of Staff or designee. The incumbent supports the medical center's mission by possessing and exemplifying the core competencies of the High Performance Model The nurse's primary commitment is to the patient/resident, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). 1. Receives and completes inpatient/outpatient consults utilizing comprehensive knowledge of clinical practice and critical care principles to assess appropriate level of care and to ensure compliance with established guidelines resulting in expedite veteran transfers . 2. Conducts clinical reviews to interpret and evaluate complex data directly related to diagnosis, condition, and treatment to assure appropriate medical care/assignments are followed. 3. Applies knowledge of adult and geriatric disease entities and current accepted treatment modalities to establish conformance with criteria for transfers. 4. Utilizes applicable technical skills maintaining tracking methods for data retrieval. 5. Demonstrates knowledge of complex automated data processing systems and their application for communication and data management. (e. g., VISTA, CPRS, VHA Intranet, Microsoft Word, Excel and Outlook) 6. Analyzes data and communicates care coordination outcomes data as requested to assess need for improvement in departmental processes. 7. Assumes leadership responsibility in providing direct support for the development, coordination, maintenance and operation for clinical care coordination within the Transfer Center at the departmental level when the Lead RN Transfer Coordinator is on leave or otherwise not available. 8. Communicates effectively both orally and in writing with professional, administrative, agencies and technical staff within the medical center and Non-VA organizations and/or providers as needed to coordinate GEC Skilled Home Health/Hospice services. 9. Promotes customer service, internal/external, by incorporating the VHA Core Values (Trust, Respect, Commitment, Compassion, and Excellence) into daily practice and performance. 10. Proficiently provides direct patient care at bedside, to include during medical transports. 11. Maintains a unrestricted Driver's license, knowledge and proficiencies of Ambulance operations, various DME, Medical equipment, and of Stryker Gurney competencies. 12. Demonstrates the ability to appropriately document in the Medical Record. Preferred Experience: ED/ Critical Care ICU 2 years experience Case management/discharge planning experience Knowledge of EMS procedures and acuity levels Knowledge base of DME Knowledge of Interdisciplinary collaboration 1 year Charge Nurse or NOD experience Knowledge of gurney, transport monitor, and critical care ground transports. Work Schedule: Monday-Friday 8 or 10 hour shifts . Shifts will be determined by the needs of the service . Financial Disclosure Report: Not required Requirements Conditions of Employment U. S. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. U. S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
