Summary The Connect Registered Nurse is the provision of integrated, accessible health care and are responsible for addressing a majority of personal health care needs, developing a sustained partnership with patients. The nurse is an integral member of the interdisciplinary care team delivering comprehensive mental health care necessary for promotion of optimal maintenance, and management of mental and physical health with substance use disorders. Responsibilities The Connect Clinic RN functions in an indirect role providing face to face services, as well as consulting or coordinating care wherever veterans are at risk or experiencing mental or physical illness. The RN provides comprehensive, trauma-responsive basic care and treatment, general health teaching, health screening, and appropriate referral for treatment of general or complex physical health problems. The RN demonstrates leadership by serving as an advocate for patients, team player to colleagues and he/she continues to enhance his/her own and the team's professional growth, development, and practice. The RN is directly responsible to the Nurse Manager for Domiciliary Nursing/Mental Health Outpatient. The registered nurse, as a licensed professional, will be responsible and accountable for his/her practice. The Connect Clinic-RN then selects and implements evidence-based or best-practice interventions to assist with the optimal achievement of the individual's recovery goals and periodically evaluates both the attainment of the goals and the effectiveness of the interventions. Specific practice settings: Nursing Mental Health; Domiciliary. The practice of each registered nurse is based on knowledge, experience, and research and has a direct impact on patient outcomes in the assigned area and for the facility. Delivers fundamental, knowledge-based care to assigned patients while developing technical competencies. Direct Care Nurse/Staff Nurse: The registered nurse is directly responsible to the designated manager in the assigned area; the registered nurse is responsible and accountable for the provision of direct patient care and for the delivery of quality nursing care to a culturally diverse veteran patient population. The registered nurse possesses the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. This enables effective implementation of: health promotion and prevention practices management of acute and chronic illness attainment of optimal levels of functioning support of the dying patient and significant other Role responsibilities and accountabilities also include demonstration of knowledge and skills necessary to provide care appropriate to the age-related needs of patients served in the assigned area of practice. This knowledge includes understanding changes associated with aging and principles of growth and development relevant to the adult and geriatric population. The registered nurse is also responsible and accountable for maintaining BLS and/or ACLS training. In addition, the registered nurse is accountable and responsible for possessing the knowledge and skills to: Communicate and interact appropriately and courteously with all internal and external customers. Maintain confidentiality of electronic, written, and/or verbal patient/employee information. Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment. Other duties as assigned. The incumbent may receive a variety of additional assignments/special projects, either to meet the needs of the Medical Center or designed for professional growth of the individual. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
