Description
Responsible for serving as the first reliable point of contact for Patients, Families and Providers.
Principal Accountabilities:
1. Serves as patient and family advocate.
2. Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care.
3. Refers patients to support services as Social Work and Dietitians
4. Assists in education of the patient/family to understand disease process and care map.
5. Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient.
6. Assists with coordination of patient care along with the physician and team
7. Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment and from inpatient to outpatient care and services.
8. Performs other related duties as assigned.
Minimum Education:
High School diploma or GED required
BSN Preferred
Minimum Experience:
1-2 years Radiation Oncology experience preferred but not required.
Licenses/Certification:
Valid New Jersey Nursing License Required
BLS Required
