Summary The Registered Nurse - Care Coordination and Integrated Case Management Co-Champion Program Coordinator is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Responsibilities The Registered Nurse - Care Coordination and Integrated Case Management (CCICM) Co-Champion Program Coordinator is administratively responsible for the clinical programming and daily operations of the Southeast Louisiana Veterans Health Care System (SLVHCS) CCICM program. Develops and implements policies and procedures specific to SLVHCS and ensures compliance with local facility, VISN, Office of Nursing Service (ONS) and Care Management and Social Work (CMSW), and VA Central Office directives and handbooks, as well as The Joint Commission, and other accrediting bodies and regulatory standards as needed. The incumbent is responsible for the planning, development, implementation, monitoring, and evaluation of program operations. Duties include but are not limited to the following: Develops and implements a facility level framework for implementation of national standardized processes and tools that integrate communication, collaboration and coordination across services and programs to expand upon existing VHA program approaches and address concerns about care coordination service delivery gaps and service duplication. Identifies cohort of Veterans across SLVHCS with the highest opportunity and highest need. Identifies potential areas where care coordination is taking place to include but not limited to primary care, surgical care, mental health, community care, specialty clinics, geriatrics, and chronic disease. Integrates inpatient and outpatient care coordinators, care managers, and case managers for seamless continuity of care. Develops, implements, and monitors process for ongoing screening of identified cohort. Identifies care coordinators and support services across the specialized services of SLVHCS and ensure continued participation by setting expectations, clear roles, and responsibilities. Develops and implements facility wide CCICM education plan to key stakeholders to include Executive Sponsor, Co-Champions, Service Chiefs, supervisors/managers, frontline staff, and Veterans with plan for ongoing sustainment education plan. Coordinates and collaborates with the facility level care coordination and case management teams to include but not limited to nurse and social worker navigators, care coordinators, care managers, and case managers. Coordinates and co-facilitates a care coordination review team (CCRT) to integrate communication, collaboration, and coordination when assigning a lead Coordinator to Veterans identified as needing moderate or complex care coordination services. Delivers fundamental knowledge-based care to assign clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Manages and controls the use of data related to these programs regarding day- to-day functioning and program evaluation. Implements standardized tools designed to integrate care horizontally and vertically to include but not limited to, bridging medical care and mental health care, transitioning inpatient care and outpatient care, and integrating community care and VA care thereby creating a seamless continuum experience for the Veteran. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
