Job Overview:
The Stroke Program Manager is responsible and accountable for assuring the delivery of quality health care services to the Stroke patients. The program supports the needs of the Stroke patients by design, management and implementation of the Stroke Program that standardizes care and facilitates continuous improvement to achieve maximum quality outcomes throughout the continuum of care. The Stroke Manager works with the interdisciplinary team in the development, implementation and evaluation of data collection, analysis and reports according to established clinical practice guidelines and standards. They will assist in professional education programs as requested and provide support and resources to the stroke patient population.
Qualifications:
Required:
RN with at least five years clinical experience and caring for stroke patients
Demonstrate analytical and critical thinking skills with pro-active decision and negotiation skills
Ability to work effectively as a team member while assuming a leadership role
Ability to analyze data and present to appropriate committees
Preferred:
Critical care experience and CCRN preferred

Essential Functions:
Develops and implements policies/ procedures and clinical practice guidelines that guide and support the provision of services related to Stroke disease processes
Coordinates the research and development of clinical outcomes data for the Stroke population
Maintains process for clinical data collection, analysis, trending and communication of results
Reports Stroke data to appropriate committees.
Responds to all Stroke Alerts while on campus and provides guidance support and leadership to the patient, loved ones and
staff
Conduct timely review on all Stroke alerts
Assists when required with the transferring of stroke patients to a higher level of care
Gathers, maintains and analyzes statistical information and assists in its interpretation of practical application
Serves as leader of the Stroke Performance Improvement Team
Coordinates with the Quality Resources Department to ensure acceptable sampling size for chart abstraction and timely communication of results of abstractions
Maintains a positive collegial relationship with department heads, physicians, and patients
Maintains clinical knowledge to assist clinicians in analysis of clinical data
Remains knowledgeable of new procedures and practices in Stroke care
Benchmarks customer satisfaction outcomes utilizing surveys and call backs.
Networks and benchmarks with professional organizations to ensure the institution is meeting standard of care
Maintains the confidentiality, security and integrity for the management of associate / patient information according to the Newark Beth Israel Medical Center policy and procedure
Serves as core member of and resource person for staff and other members of the STROKE Team
Networks with community organizations regarding Stroke education
Collaborates with appropriate affiliates to leverage opportunities and provide seamless customer relationship management
