**Transitional Care Manager**

**Hospice and Home Health**

**Registered Nurse**

**Remote Opportunity w/ 10 % travel**

The **Transitional Care** **Manager** is responsible for overseeing the overall transitional care duties, operations, goals, and outcomes of progress while acting as a liaison between service lines. The **Transitional Care Manager** assists all employees and clients as the care plan progresses through the continuum of care. This leadership position involves managing daily operations, coordinating patient care activities, and promoting a positive and collaborative work environment. This position is responsible for the oversight of the Transitional Liaison role that identifies potential end-of-life and alternative patient care needs for skilled home health patients, assesses patients for clinical appropriateness, hospice eligibility, Palliative Care consult opportunities, and serves as an educator to internal and external stakeholders regarding hospice benefits and the overall transition process between services. The Transitional Care Manager will work closely with service line leaders to enhance patient outcomes and maintain compliance with regulatory standards.

As the Transitional Care Manager, you'll contribute to our success in the following ways:

Acts as liaison and consultant for patients, families, and caregivers who may require assistance with advance care planning and leads the team to provide this assistance.

Executes strategic plans and strives to obtain departmental goals.

Achieves goals by ensuring there is adequate, professional, scheduled staff in the facilities to provide the best care possible.

Develops and implements day to day program procedures and practices. Communicates changes to these procedures and practices in a timely manner as the program grows and evolves.

Acts as a consultant between home care and hospice related to potential hospice eligibility, Palliative Care consult opportunities.

Develops policies and procedures to enhance the Transitional Care program.

Provides leadership for department and staff.

Serves as an educator to internal and external key stakeholders, a resource person, and assists in orientation of all new hire employees.

Responds to concerns from co-workers, including program clinicians, and payer inquiries regarding invoices in a positive friendly manner.

Maintains a thorough understanding of applicable insurance rules and regulations, on program financial situations, goals, budget, and outcomes. Continuously looks for cost savings opportunities.

Produces dashboards and data to executive staff for tracking outcome of the program.

Responds to patient and/or family complaints and works to resolve them in accordance with policy.

Treats all patients and caregivers with respect and courtesy.

What is Required?

Bachelor's Degree Nursing preferred

3 years nursing experience in hospice, home health, geriatrics, or general med-surg nursing

Certified Hospice and Palliative Nurse (CHPN) or willingness to obtain the certification within 2 years.

Must have a dependable vehicle, valid drivers license and current auto insurance in accordance with the laws of the state.

You will report to the Director of Care Transitions.
