Position Summary: The purpose of this position is to nurture and support Elders by providing excellent quality of care in a safe and closely associated family environment. Responsibilities include:
Recognize and respond to the needs of the Elders.
Provide and document licensed nursing interventions and treatments as ordered by the physician in accordance with standard nursing practice, DOH regulatory requirements and facility policy.
Notify RN, Case Manager or on-call to report changes in health status of Elders, schedule appointments, clarify information and/or obtain medication/treatment orders, communicate with pharmacist.
Provide direct care to Elders as necessary including personal care, medication administration, assistance with transfers/ambulation, etc.
Monitor and oversee individual medication regimes, report side effects and adverse reactions to RN and Physician.
Support RN Case Manager with ongoing monitoring and supervision of the facilities medication management plan.
Respond to Elder and family questions and concerns.
Conduct investigations related to complaints and grievances, provide documentation and appropriate follow up.
Implement new administrative or clinical interventions and/or protocols to help prevent issues from reoccurring.
Mentor, support and collaborate with the Certified Home Health Aides.
Perform other duties as assigned.
Qualifications:
A current license as a LPN in good standing with NYS DOH is required.
A minimum of 2 years LPN work experience preferred.
Home Health or Long Term Care experience preferred.
Good verbal and written communication skills.
Physical Requirements:
Standing, walking, sitting, climbing stairs, lifting, and ability to assist Elders.
