Brendan House Long Term Care is looking for an LPN Resident Care Manager to join their team!

This position assumes a visible leadership role and is responsible for overseeing patient/resident care, administrative activities and care delivery for assigned area(s). Oversees day-to-day operations and continually evaluates quality and effectiveness of patient/resident care, clinical judgment, productivity, staffing, and education for assigned area(s).
Qualifications:
Current Montana LPN License or a multi-state compact license with authorization to practice nursing within the state of Montana; required.
Current Healthcare Provider CPR required.
Minimum of one (1) year current unit specific experience preferred.
Previous Charge Nurse, Supervisor, Geriatric, Medical-Surgical experience preferred.
Experience with nursing information systems, Medicare billing, and Skilled Nursing Facility Patient Driven Payment Model (PDPM) preferred.
Possess knowledge of the health care environment, long-term care regulations, and care delivery systems for assigned area(s) preferred.
Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Commitment to working in a team environment and maintaining confidentiality as needed.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.
Job Specific Duties:
Assumes a visible leadership role and is responsible for overseeing patient/resident care, administrative activities, and care delivery for assigned area(s). Oversees day-to-day unit operations as applicable to assigned area(s).
In partnership with leadership, contributes to hiring, corrective action and termination decisions, conducts annual performance evaluations, provides feedback to employees regarding performance, clinical judgment, interpersonal and communication skills of assigned area(s).
Promotes staffing compliance and ensures safe staffing assignments. Monitors staffing to achieve productivity and budget standards.
Performs within the scope and standards of practice for Licensed Practical Nurses in the state (as set forth by the current State Board of Nursing regulations, the American Nurses Association, and professional organizations specific to specialty to guide practice and standards within the workplace).
In collaboration with assigned leadership, develops, revises, and ensures compliance with policies and procedures, standards of care, and professional practice acts as applicable to assigned area(s).
Serves as a role model by providing leadership expertise and serves as a clinical and administrative resource. Provides coaching and development opportunities for assigned staff as applicable to assigned area(s). Works with Director of Nursing and/or RN Resident Care Manager to provide clinical oversight to assigned staff per protocol.
Serves as an advocate for the professional nursing practice and supports the work of the unit-based council. Works in conjunction with the assigned leadership to advance the practice of nursing and the delivery of care in a coordinated, supportive way.
Coordinates delivery of patient care in a professional manner in compliance with long term care and nursing policies and procedures.
Determines patient/resident assignments and delegates' duties according to appropriate patient acuity and skill mix of staff.
Communicates with assigned leadership and documents any events that occur.
Initiates, coordinates, and oversees the Resident Assessment Instrument (RAI) process for assigned area(s). Completes the nursing elements of the Minimum Data Set (MDS) 3.0 and provides staff orientation and education as needed. Implements and/or assists with plan of care. Facilitates care conferences for assigned area(s). Coordinates with Director of Nursing and/or RN Resident Care Manager to sign off on MDS tool.
Initiates and coordinates processes to maximize reimbursement and assures compliance with federal and state guidelines. Ensures appropriate utilization of the Patient Driven Payment Model (PDPM) system. Sets Assessment Reference Dates (ARDs) to optimize the PDPM system. Facilitates and/or participates in Medicare meetings.
Adheres to State and Federal rules and regulations concerning delivery of patient/resident care and assures nursing services are provided and documented accordingly.
Participates in the safety, quality, and risk management programs to improve structure, process, and outcomes of care. Participates in organizational committees/task forces as assigned. Prepares for, and collaborates with, other stakeholders prior to and during the CMS and long-term care survey.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
