Description
The Physician will be appointed to the medical staff and assigned to the Geriatrics Rehabilitation and Extended Care Service (GREC) patient care line, Home Based Primary Care (HBPC) and Hospital in Home (HiH) programs. The Physician is academically and clinically able to perform a broad spectrum of patient care services at the expert level. In addition to providing primary and geriatric health care, the Physician serves as a consultant to nursing and other interdisciplinary team members. The Physician is eligible to serve on medical center, VISN, national committees, and to assist in the development, evaluation, and communication of policies and standards to all healthcare disciplines.
The major duties and responsibilities include, but are not limited to:
- Provides patient care directly or through the supervision of fellows/residents in either the home or outpatient settings.
- Effectively interface with hospital and Ambulatory Care staff about program elements that will facilitate appropriate referral and care of the home care veterans.
- Provide direct patient care by completing comprehensive assessments, including diagnosis and treatment of a range of medical, geriatric and palliative health conditions.
- Initiate and adjust therapeutic regimens, perform histories and physicals, order diagnostic and therapeutic tests/studies, initiate consults and referrals, and prescribe medications/supplies in accordance with VA policy, VA formulary, Federal & state regulations, and scope of practice.
- Provide clinical input and oversight for all patient treatment plans for HBPC and HiH.
- Provide coverage for other Geriatrics and extended care (GEC) programs as determined by the ACOS for Geriatrics and extended care.

Work Schedule: Monday - Friday, 7:30am - 4:00pm
Pay: Competitive salary, annual performance bonus, regular salary increases
