Estimated hiring range $ 68,000 - $83,000/year, 5% bonus target, full benefits.

Posting Notes

This hybrid position works Friday to Monday, 10-hour shifts. 75% of work will be done remotely, providing telephonic nursing triage, and 25% of work will include seeing patients in the field.

Job Summary

This position is responsible for providing clinical triage support to patients, families, and caregivers as well as supporting the hospice field staff and informing the clinical staff if a home visit is needed. This position also reviews clinical documents including medication records and referral information as part of the hospice intake process as well as providing phone support in the hospice office as needed. This position will also include support for the field nurses when needed, including phones calls, and focused visits, under the direction of a RN and per the plan of care.

Essential Responsibilities

+ Manages all incoming phone calls for hospice.

+ Under direction of an RN, reviews all incoming referrals and collaborates with clinicians to gather necessary information to determine appropriateness for hospice services. Collaborates with Medical Directors and hospice leadership as needed.

+ Collaborates with RN and other members of the hospice office regarding referrals to ensure timely intakes are scheduled.

+ Provides telephonic triage support to patients, families, caregivers as needed for Hospice field staff and advocates for patient needs through support and communication with hospice IDT.

+ Assists in reviewing and assigning RN case manager and Hospice team to new referrals.

+ Under direction of an RN, makes patient visits as needed for back-up and support.

+ Gathers and reviews medical records and medication list for accuracy.

+ Responsible for documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards according to policies and procedures.

+ Assists with hospice chart reviews, as requested.

+ Orders DME, medications or supply refills through PBM/3rd party vendor, as appropriate and in line with the practice standard.

Experience and/or Education

Required

+ Graduate of an accredited LPN training program

+ Current unencumbered LPN license in the State of Oregon

+ Minimum 1 year experience in hospice, palliative care/oncology, geriatrics and/or home health

+ Experience providing triage support for patients

+ CPR certification at hire or within 6 months in position

Preferred

+ Minimum 2 years' experience in hospice, palliative care/oncology, geriatrics and/or home health

Knowledge, Skills and Abilities Required

Knowledge

+ Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations

Skills and Abilities

+ Demonstrates ability in case management, nursing and counseling in a home-based setting

+ Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation

+ Must have general computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet

+ Excellent interpersonal skills

+ Ability to tolerate and manage grief

+ Ability to work both independently and collaboratively
