Job Description Position Title: Registered Nurse (RN)

FLSA STATUS: Non-Exempt

REPORTS TO: Nursing Manager

SUPERVISES: LPNs, Home Health Aides

DEPARTMENT: Home Care

COMPANY: OVNA

6-Month paid preceptorship/mentorship program for Massachusetts licensed RN's to learn home care.

POSITION SUMMARY :

The Registered Nurse plans and provides individualized nursing care and comprehensive services to home care patients. The Registered Nurse assists in clinics and other agency programs as assigned by the Nursing Manager. The Registered Nurse is expected to meet established productivity standards (to include weekends/holidays as needed) as well as a rotation of on-call responsibilities.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

Assesses patient's health problems and needs and develops plan of care.

Provides direct skilled nursing care as per physician orders.

Observes patients, charting and reporting changes in patient's conditions, such as adverse reactions to medications or treatment and taking any necessary action.

Document medical records and prepare other reports as needed.

Educates patients and families in self-care and health management.

Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.

Provides case management when appropriate.

Attends case conference as requested.

Adheres to policies and procedures.

Participates in agency quality management and safety activities.

SUPERVISORY RESPONSIBILITIES

Completes home health aide supervisions and orientations as needed.

Co-signs orders and provides oversight of care for LPNs.

Educational Requirements and Experience:

Graduate of an accredited school of nursing

Current license to practice as a registered nurse in the state of Massachusetts

Current CPR required

One year nursing experience, no home care needed

Current MA driver's license and reliable transportation

Knowledge, Skills and Abilities:

Language Ability: Ability to read, write, speak and understand the English language. Knowledge of the structure and content of the English language including the meaning of words, rules of composition, and grammar. Ability to effectively communicate with peers, supervisor, residents, clients, and guests.

Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measurement, using whole numbers, common fractions and decimals.

Cognitive Demands: Ability to apply common sense understanding to carry out instructions provided in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Able to swiftly change direction as necessary.

Computer Skills: Ability to use Microsoft Windows, email and the Internet. Ability to use software/device for data entry of visit documentation.

Competencies: As outlined in orientation plan.

ENVIRONMENTAL ADAPTABILITY

Works primarily indoors in varying climate-controlled settings

Possible exposure to unpleasant odors

Possible exposure to chemicals as identified in the MSDS Manual

Subject to hostile and emotionally upset family members, personnel and visitors

May work beyond normal working hours and on weekends and holidays when necessary

Required to travel to multiple work sites and up to 200 miles per day (Productivity is adjusted to accommodate travel - every 100 miles traveled in 1 day = 1 productivity point

POSITION REQUIREMENTS:

PHYSICAL REQUIREMENTS/WORKING CONDITIONS :

Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 35 pounds.
