**Primary Responsibilities:**

Directly supervises RN Case Managers, Social Workers, Case Management Assistants and Emergency Department Case Managers, ensuring compliance with Optum, BCH standards and Joint Commission standards

Coordinates all the case management functions for the hospital

Responsible and accountable for coordination of patient services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care

Through concurrent case management and utilization review patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care

Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization.

Quality and Risk Management issues will also be monitored for and reported as appropriate

**Required Qualifications:**

Current Colorado RN or Social Work license

2+ years of case management experience in area of specialization

Reside near Boulder, CO with the ability to work onsite 5 days a week

**Preferred Qualifications:**

2+ years demonstrated success in healthcare analytics and/or consulting

2+ years of experience with operational processes and clinical decision making in the various providers within a health care delivery system?(e. g., hospitals, medical groups, post-acute care organizations, physician practices, ambulatory surgical centers, etc.)

2+ years of experience leading analytics projects or initiatives with minimal guidance

1+ years of experience guiding and developing junior team members

Demonstrated strength of analytics competency using, Excel, SQL, or another comparable analytical tool

**Colorado Residents Only:** The salary range for this role is $70,200 to $137,800 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
