IMMEDIATELY HIRING: Registered NurseJob Description:
Under the direction of a Nurse Manager, the nurse provides skilled care and routine clinical nursing assessment visits to members and families. This role involves utilizing all elements of the nursing process in compliance with applicable laws, regulations, and agency policies. As a core interdisciplinary team member, you will be assigned to a member in compliance with regulatory requirements.
Key responsibilities include carrying out the agency's mission, philosophy, goals, and objectives; interpreting and implementing the Agency's philosophy to staff and members of the community; assuming responsibility for assignments given, and seeking supervision appropriately.
Administers medication and treatment as prescribed by the PCP.
Investigates and assists in documenting and resolving customer issues and grievances in a timely, accurate, and courteous manner. This includes working with other departments to resolve the source of the problem(s).
Provides prescribed or emergency care to patients requiring professional nursing services, including: venipuncture, dressing changes, straight catheterization, suctioning, immunizations, etc.
Performs diagnostic tests as authorized by the Primary Care provider (PCP). Obtains laboratory specimens when indicated. Provide treatment under medical direction.
Assesses, evaluates, and reports to physician the participant's physical and emotional condition, reaction to medications and/or treatments, and/or critical laboratory result requiring PCP intervention.
Independently perform skilled nursing procedures, assessments and techniques based on specific Participant's needs while at the PACE Center.
Only act within the scope of the individual's authority to practice.
Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
Skills & Qualifications:
1+ years of general clinical nursing experience in a certified home health agency, acute care, medical surgical, and/or critical care
Active New York State registered nursing license
Current Driver's License, required insurance and car available for work
Ability to participate in mandatory PACE comprehensive care planning meetings routinely
Collaboration skills for developing a comprehensive care plan based on member-specific needs, physician orders, UAS-NY CAPS, other IDT assessments, members' specific parameters and identified goals
