POSITION SUMMARY

Key Medical Care, A SilverStone Health Company, is seeki ng Discharge Coordinator. The Discharge Coordinator performs administrative duties under the guidance of the Director of Operations.? This position is responsible for handling the coordination of patients transitioning from a medical facility to their home.

At SilverStone Health we are reimagining the at-home continuum of care. We guide, empower, treat, and support patients and families facing a chronic or terminal illness, enabling them to make the most informed decisions throughout their healthcare journey.

NOTE: Applicants must reside in Texas - preferably in the DFW area

ESSENTIAL DUTIES AND RESPONSIBILITIES

Include the following. Others may be assigned.

Evaluates patient census and coordinates patients transition from a medical facility to their home

Communicates with patient and patient's family regarding transition from facility

Facilitates remote patient monitoring (RPM) for in-house Anelto vital signs monitoring

Conducts post-visit patient follow-up calls to remind patients of their follow-up visits

Monitors patient charts to ensure patients are being seen by scheduled providers

Partners with CCM Coordinator and RNs to help coordinate care of patient

Maintains security of medical records and patient's health information according to HIPAA standards

QUALIFICATIONS

Excellent analytical, organizational, and verbal/written communication skills

Detail orientation

Ability to manage multiple priorities with varying levels of complexity.

Strong customer service orientation

Proficiency with MS Office applications and the internet

Ability to document using proper medical terminology in EMR system

Ability to read, write and speak English proficiently

Knowledgeable of and maintains HIPAA standards of privacy and confidentiality. Ability to use discretion appropriately and maintain confidentiality

Strong listening skills and ability to show compassion for a diverse patient population and their families.

Education / Experience: Include minimum education, technical training, and/or experience preferred to perform the job.

Required:

1-2 years of work-related experience in healthcare

High School Diploma or GED

Preferred:

2-4 years of work-related experience in healthcare

Experience with Chronic Care Management

Bachelor's Degree in Healthcare

Frequently required to stand, walk, sit, use hands to feel, and reach with hands and arms, includes ability to walk through skilled nursing facilities.

Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to: remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)

Light to moderate lifting 20-25 pounds

Fine hand manipulation (keyboarding)

The noise level in the work environment is usually low

Work is normally performed in a typical interior work environment which does not subject the employee to any hazardous or unpleasant elements.

Position will need to adhere to any immunization, vaccination, or other requirements in place at our client sites in accordance with their policies and procedures.

PAY:

17 USD per hour

This particular role can be performed remotely
