JOB PURPOSE:

The DTC - RN Supervisor will oversee the daily operations of a diagnostic and treatment center and provide leadership and supervision to the nursing staff through reinforcement of quality and safe delivery of nursing service. The role is also concerned with the strengthening of the nursing practice, the design of improved clinical workflows, and in general system improvement. They may provide backup clinical DTC coverage and will support the professional development of the DTC nurses. They are the stewards of NCQA (National Committee for Quality Assurance) and the adoption of the PCMH (Patient Centered Medical Home) model principles.

JOB RESPONSIBILITIES:

Clinical Oversight

Works with various platforms and stakeholders to facilitate the participant's hospital admission and emergency room follow up.

Helps handle the rising clinical issues of the day and Support developing care plans.

Assists in conducting triage or gathers information regarding urgent patient needs and communicates with provider to resolve patient care issues promptly.

Review and update policies concerning nursing practice.

Provide education and support nursing professional development accordingly.

Collaborates with care management and community health nurses on complement their work on the subset of the population that visits the PACE sites.

Participates in morning reports and other IDT meetings regarding the management of our outcomes of care and the handling of high-risk members.

Provide clinical support for daily DTC needs and site related events ensuring appropriate DTC RN coverage.

Only act within the scope of the individual's authority to practice.

Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.

Administration

Serve as the liaison by collaboration between the DTC and PACE IDT, and community medical groups on a needed basis.

Participate and effectuate workflow standards related to FTC lab work including communication with third party vendors as needed.

Provide leadership and supervision to the nursing staff in the DTC, including hiring, training, and scheduling.

Work with care delivery leadership to develop and maintain policies and procedures to ensure the provision of high-quality, patient-centered care in the DTC.

Manage the inventory tracking of medical supplies, pharmacy, and equipment and ensure that they are properly maintained and in compliance.

Collaborate with care delivery leadership on all efforts pertinent to improving the collective nursing practice in the organization.

Reviews and updates DTC manual related to DTC RN functionality to ensure standard are up to CMS and PACE regulation standards.

Quality Improvement and Population Health

Assists in promoting and implementing PCMH initiatives.

Regularly participates in leadership, nursing staff, and other monthly/quarterly quality improvement meetings to educate staff about PCMH and population health initiatives.

Develops and maintains a trackable spreadsheet registry of high-risk patients in collaboration with patients, families, and providers.

Develops and implements ongoing processes with Nurse Care Managers to determine level of care coordination support that patients and caregiver(s) need and/or desire.

Collaborate with care delivery leadership to develop and implements workflow in specialized populations, such as diabetes, cancer screening, or payor associated QI projects.

Develops and implements ongoing processes with Leadership, Nurse Care Managers, and other departments to improve DTC quality metrics and the methods for measuring and reporting on them.

Regulatory Readiness

Participates in policies and procedures revision on a routine basis.

Ensure DTC site readiness for CMS survey across all sites.

Schedule: 8:30AM - 5:30PM

Weekly Hours: 40

QUALIFICATIONS:

Education:

Degree from an NLN-accredited school of nursing required, BSN preferred.

Successful completion of an accredited Nursing Program is required.

Baccalaureate degree is highly preferred.

Completed NYSED-approved infection control coursework. A minimum of 8.0 education contact hours that may include contact hours of mandatory training requirements such as infection control coursework.

Experience:

Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

License:

Current active and unrestricted license and registration in NYS as a Registered Nurse, required.

The following are required certifications at the time of hire or transfer and must demonstrate recent verifiable experience (within one [1] year) to independently perform:

Phlebotomy / Venipuncture

IV Therapy

EKG Interpretation

Basic Life Support certified by the American Heart Association or American Red Cross

Additional Requirements:

Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function.

Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

Self-directed with the ability to work with or without supervision.

Proficient in the use of a computer for email, word processing, research, and maintenance of Participant records

Demonstrated ability to establish a good rapport with participants and peers is essential.

Must be able to work weekends.

Excellent written and verbal communication.

Works inside at office locations and outside in varied weather conditions in all areas of the community, using private or public means of transportation. Possess and maintain good physical stamina and mental health, including satisfactory health clearance as required by NYS regulations and Agency policy.

Language: Bilingual preferred.

Physical Requirements

Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to:

Standing - Duration of up to 6 hours a day.

Sitting/Stationary positions - Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods.
